Please print and fill out this form (Volunteer Waiver and Background Check) and bring it with you when you come to volunteer with us. You can also pick up a printed copy from our offices.

Volunteers from the Texas State University course, “Social Services in the Community” (SWK2375), should review the following documents as they contain information they will need to complete course assignments.
